For parents & educators

Notice progress. Start a helpful conversation.

Enter the child’s date of birth and this tool will choose an age-appropriate set of observations. Work through one item at a time, then print a clear summary.

  • Private by design — nothing is uploaded or saved
  • 12 age bands — from 2 months to 5 years
  • Five developmental areas — 15 short observations in each check-in
  • No pass or fail — just useful observations and notes

Step 1 of 3

Find the right age set

Only the date of birth is required. The optional details make the printed summary easier to identify.

How it was created

Original questions, informed by public milestones.

Observe, don’t perform

Use recent examples from ordinary routines. A child does not need to demonstrate every skill on demand.

Consider the whole child

Culture, home languages, opportunity, health and context can all affect how a skill is shown.

Act on concerns

This tool supports conversation. Only validated tools administered appropriately can provide developmental screening.

Content model informed by the CDC developmental milestones and American Academy of Pediatrics guidance. The five-area layout and “Yes”, “Sometimes” and “Not yet” choices provide a familiar format, but every observation is independently written and no licensed ASQ-3 questions, scoring thresholds or interpretations are used. This tool is not affiliated with or endorsed by ASQ-3, the CDC or the AAP.
